How do I enter a referral in the SWIS mobile app?

  1. Log into SWIS Mobile. 
  2. Tap the New Referral button in the bottom right-hand corner. 
  3. Choose the referral type: Administrator-managed for behaviors you send out of the classroom, or Staff-managed for behaviors you manage in the classroom. 
  4. On the first page, enter the student’s namegrade level, the referring staff name, location, date and time…Coincidentally, these are all the same fields required to save a draft referral.  
  5. Tap Next.
  6. On page 2, enter the behavior or behaviors, the motivation and others involved. 
  7. At this point, you can either send the referral for review or continue to fill out the rest of the form by clicking Next. 
  8. If we tap Next we enter any action taken, seclusion/restraint information, and  any notes we want to include about the incident. 
  9. Tap NextIf your school has Custom Fields, this is where you'll enter them.
  10. Tap Next to review the referral. Make sure all the information you entered is correct and either request a review or click Save.
